Services for all Florida Death Certificates

     BROWARD COUNTY, FL — Ordering Death Certificates from the Broward County Health Department is now easier and more convenient than ever.  Effective Monday, July 16, 2012, application for Death Certificates for ANY death occurring within the State of Florida, during 2012 and in the future, will be available online through the Florida Electronic Death Registration System (EDRS) on the Broward County Health Department’s website – www.browardchd.org. Certificates for Florida Births are still available through the regular Vital Records system on the Broward County Health Department’s website. 

     “We are delighted that this service will be made available to the public,” said Paula Thaqi, M.D., M.P.H., Director, of the Broward County Health Department.  “Through this online system and our usual walk-in, and phone ordering, people will have a more convenient way to access death certificates for out of County deaths,” she said.

     Previously, the public could access records for Broward deaths only – all other records would be available within the county of death or through the Florida State Office of Vital Statistics in Jacksonville. The EDRS will also allow for easy access for physicians and funeral directors when certifying and recording information on death records.

     The public can simply visit www.browardchd.org, click the “Birth/Death Certificates” button, and then follow the instructions for completing the form.  Along with the cost of the certificate(s), a shipping and handling fee is required.  Express service is available for an additional fee.

     Walk-in and phone service is still available at Broward County Health Department, Office of Vital Statistics, 2421A S.W. Sixth Ave., Fort Lauderdale, Fla. and 7999 Pembroke Road, Hollywood, Fla.  The offices are open Monday through Friday, from 8 a.m. to 4:30 p.m.  To order by phone call toll free (866) 830-1906.

     The Office of Vital Statistics provides information on birth and death certifications only.  The public should be reminded that the office does not handle marriage licenses, divorce, probate, wills, or social security.
