Community Forum: Protection and Affordable Care Act
The Broward County Alumnae Chapter of Delta Sigma Theta Sorority, Inc. in collaboration with Broward Health and the Broward Community and Family Health Centers will present a community forum on the Patient Protection and Affordable Care Act, also known as Obama Care. This unique and interactive community forum will also provide individuals and families with an opportunity to complete an application and enroll in an affordable health insurance plan of choice.
The event will be held on Saturday, Jan. 25, 2014 at 10:30 a.m. to 3 p.m. at the KC Wright Life Development Center (Delta House) which is located in the Osswald Park, 2220 N.W. 21 Ave., Fort Lauderdale, Fla. Participants are encouraged to come prepared to select and enroll in an afford-able health insurance plan offered through the federally facilitated Health Insurance Market-place. Broward Health and the Broward Community & Family Health Centers combined their resources to provide Certified Application Counselors (CACs) and Navigators onsite to facilitate outreach, education and enrollment.
Participants should bring the following information to assist with completing applications for enrollment:
· Social Security Numbers for every member of your house-hold who needs coverage
· Income Information for every member of your household who needs coverage (examples, Pay-Stubs, W-2 Forms or Tax Return)
· Policy Numbers for any current health insurance plans covering members of your household
· Employer Insurance Information
